Next Steps: Coming in for a Landing - Closing and Submitting Stipend Paperwork

Mentor Invoices & Mentee Request for Stipend
Once all the steps outlined on the stipend request paperwork below are complete, send your stipend request along with supporting documentation via fax to Lenora Bruce at 530-752-6135. All invoice and stipend request paperwork must be submitted by Friday, June 5, 2009.

Mentees: To download the Mentee checklist and request for stipend click here.

Mentors that submitted an invoice for ½ of their stipend earlier in the year:
You will need to submit an invoice and paperwork to receive the second ½ of your stipend. To download an invoice for the second ½ of your payment, click here.

Mentors that have not yet submitted an invoice for stipends:
You will be submitting an invoice for full payment in the project. To download invoice for full payment, click here.

Deliverables Completion Form
Some mentors and mentees have been tracking their progress via paper and using ed tech profile. Feel free to utilize the Deliverables completion form to document your progress. Submit the deliverables completion forms with your stipend request forms. Delierables completion forms do not take the place of stipend request forms, but can be used as supporting documentation. Log in to the Ed-Tech Profile Mentoring System
The online system that you will utilize for the project, along with your unique username and password is http://www.edtechprofile.org Log in to take your pre-survey and begin the project.

Locate and contact your Mentees
A graph that lists all mentors with their mentee matches and contact information can be found at: http://www.afterschoolnetwork.org/node/6976

Sample Communication Schedule
The success of your mentoring relationship may hinge on clear structured and scheduled communication. Communication does not work well if you leave it to chance. Setting a structure for communication will be essential. Please be realistic about the dates and times that you schedule to ensure you are truly available. Once you establish a communication schedule, stick with it. A sample communication schedule is provided below:

-1st Tuesday of the Month: Mentee emails mentor a sample status report. To view a sample status report go to http://www.afterschoolnetwork.org/mentee

-Mentor Logs Communication and Responds to Mentee Email: Mentors can copy and paste this communication into their ed-tech profile to record this regular communication. This will be helpful in reporting for stipends.

-3rd Tuesday of the month: The Mentor responds to mentee status report and offers additional feedback and resources with a phone conversation.

Recorded Training on the Online Ed-Tech Profile System
These trainings offered an outline of the project, the project timeline, and instructions on how to use the online system you will be utilizing multiple times monthly throughout the project.

A letter to your supervisors
It will be important to keep your supervisor in the loop on the project so that they are not surprised by your activities and can provide adequate coverage etc. Attached at the bottom of this page is a letter that will explain the project timeline and offer your supervisor the opportunity to join the mailing list for the program. This will educate your supervisors on your activities and the resources that are available to you through the project. The California After School Programs Quality Self Assessment Tool
This is a useful tool that you can use to assess your program quality and plan for program improvement and assess future technical assistance and training needs. To access the tool go to www.afterschoolnetwork.org/qsatool
