Youth Nomination Form

We would also like to take this opportunity to celebrate some of the youth, staff, parents, and sites who have made significant contributions to their program and/or community.

Criteria:

  • Service: Awarded to the youth who gave back to their community in the most significant way as part of their afterschool program (i.e. volunteered, went above and beyond in a service project, etc)
  • Leadership: Awarded to the youth who showed strongest positive leadership at their afterschool program (i.e. a strong role model, led a project, activity, club, etc)
  • Fine Arts: Awarded to the youth with the most significant contribution or standout performance in the fine arts at their afterschool program

Nomination forms must be received by September 30th!
