Parent Nomination form for Outstanding Involvement

We would also like to take this opportunity to celebrate some of the youth, staff, parents, and sites who have made significant contributions to their program and/or community.

Nominations may come from staff of any site, youth, or community members
Criteria:

• Outstanding Involvement: Awarded to the parent who has demonstrated the most outstanding involvement with and afterschool program (This should be volunteer/unpaid involvement)

Nomination forms must be received by September 30th!
