Site Nomination form for Community Service
We would also like to take this opportunity to celebrate some of the youth, staff, parents, and sites who have made significant contributions to their program and/or community.
Nominations may come from staff of any site, youth, or community members
Criteria:
- Service: Awarded to the site with the most significant service contribution to their community (Community can be school site, neighborhood, or even city)
Nomination forms must be received by September 30th!
