Cooking class permission slip

We are starting an after school cooking class in a few weeks at our high school. Does anyone have a permission slip or notice you sent home to parents to let them know students will be using knives, hot stoves, etc. that you could share?

I would love to see what you have. We do a healthy habits program where we make easy snacks for the younger kids, but i would like to incorporate the older tweeens and use more kitchen utencils, that might not be safe for the younger ones.
